Waiver / Terms & Conditions I, parent/guardian of the Student, do hereby consent to my/his/her participation in Jimmy Tagliere Golf programs (each a “Program(s)”) operated by Jimmy Tagliere (the “Company”). I acknowledge that the Lesson requires that the Student participate in strenuous athletic training and that the Program may require that my Students travel in golf carts and other motorized vehicles while participating in a Playing Lesson with Jimmy Tagliere. I acknowledge that participation in a Lesson may expose the Student to the possibility of injury. I grant Jimmy Tagliere the authority to obtain emergency medical treatment as necessary in the event the Student suffers an injury while participating in a Lesson. I hereby certify that I know of no medical problems that would increase the risk of illness and injury to me (the Student) as a result of my/his/her participation in a Lesson. By purchasing any Lessons, I acknowledge being informed of the risks associated with the Lesson's activities. By purchasing any Lessons from Jimmy Tagliere, I assume all risk for the health and well-being and hold the Company (Jimmy Tagliere), as well as its owners, employees, and other authorized agents and independent contractors, harmless therefrom. Waiver and Release: I waive any claims, demands, causes of action or any claims for relief whatsoever against, and release the Company, as well as any of its owners, employees, or other authorized agents and contractors from any and all liability, claims and/or causes of action that I may have for injuries or other damages, arising out of participation in a Lesson. Indemnification: I accept financial responsibility for any injury that I may may cause either to his/herself or to any other participant due to his/her negligence while participating in a Lesson. I agree to indemnify and hold harmless the Company (Jimmy Tagliere), as well as any of its owners, employees, or other authorized agents and contractors from liability for the injury or other consequences of any injury to any person and damage to any property that may result from my/my child's negligence while participating in a Lesson. Media Waiver: I hereby grant the Company permission to use a photograph/video image of my Student in any and all publications and media sources of the Company, including the Company’s web site, without payment or any other consideration in perpetuity. The Company will not divulge any names without written permission. Rain and Cancellation Policy. I acknowledge that a Program day or class may be canceled at the discretion of the Company due to weather or other conditions. Canceled class credit policies change per Lesson. I acknowledge that I am aware that the Company does not refund any fees for canceled Program days or classes. For Private, Semi-Private and Playing Lessons: I acknowledge that there is a 24-hour cancellation policy. Failure to give 24-hour notice will result in the full charge for the session. I have carefully read this Agreement and fully understand its contents. I am aware that this is a release and waiver of liability and I agree to it knowingly, voluntarily, and of my own free will.
